About the Protocol

Purpose of the Protocol

To support successful transition from child and youth mental health sector to adult mental health sector for young people with significant mental health and/or addiction issues

Client Population or Eligibility Criteria (eg., age; disability)

Young people, aged 16 to 24 years, with mental health and/or addictions issues; who require complex and specialized care; and who are transitioning from child and youth mental health sector to adult mental health sector
