About the Protocol

Purpose of the Protocol

To support a planned and coordinated transition from childhood to adulthood for young people who have a developmental disability, aged 14 years and older
To promote collaborative planning between school board, community agencies and families
To help prepare young people to leave children's services and when appropriate, apply for adult services

Client Population or Eligibility Criteria (eg., age; disability)

Young people, aged 1f4 years and older, who meet the definition of having a developmental disability under any of the Ministry of Education, Ministry of Children and Youth Services and Ministry of Community and Social Services legislation frameworks or agency criteria

Catchment Area or Locale

South East Region including:  Hastings, Prince Edward, Lennox & Addington, Frontenac, Leeds & Grenville, and Lanark Counties
